Process Overview

Our streamlined approach to patient care management is designed to simplify the process for healthcare providers while maximizing efficiency and revenue. Below is an overview of our five-step process that integrates seamlessly into your practice, ensuring that both patients and providers receive the highest level of care and support:

Step 1: Identify Eligible Patients

We will collaborate with the provider to pinpoint eligible patients for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services.

Step 2: Enroll Patients

We assist with recruiting, enrolling, and obtaining patient consent. For CCM, an initiating visit is required if the patient hasn’t been seen in the last 12 months. RPM patients must have an established relationship before enrollment.

Step 3: Customize Care Plans

We develop personalized care plans based on evidence-based guidelines under the supervision of in-house physicians. Providers only need to review and approve the plan.

Step 4: Manage Patient Care

Our care managers regularly contact patients, track progress using our platform, and log time toward billing thresholds. Providers are promptly notified of any patient issues through their preferred communication methods.

Step 5: Ensure Reimbursement

At the end of each month, we generate a comprehensive report outlining all billable codes for each patient, ensuring seamless and accurate reimbursement.
