Chronic Care Programs
TriageIQ™ | Multi-Domain Pre-Screeners
Chronic Care Programs
Scree. Test. Treat.
Every DTC program begins with the same clinical principle: identify risk earlier, confirm it when appropriate, and guide the patient toward timely care.
Screen. DTC starts with TriageIQ™, our proprietary universal chronic-risk pre-screener designed to identify neurological, cardiometabolic, behavioral, functional, and lower-extremity risk signals at intake. In just minutes, TriageIQ™ helps providers uncover patient-reported concerns that may otherwise go undetected during a routine visit.
Test. When risk signals are identified, providers may determine whether focused, non-invasive testing is clinically appropriate. DTC supports rapid point-of-care assessment pathways for neuropathy, vascular disease, cognitive risk, cardiometabolic risk, balance concerns, wound risk, and other chronic-disease indicators. These testing pathways are designed to support documentation, medical necessity, and payer-aligned clinical workflows when applicable.
Treat. Once risk is identified and documented, DTC helps clinics connect patients to appropriate next steps. This may include therapy pathways, care management, monitoring, patient education, referral, wound-care support, chronic-care coordination, or continued follow-up based on the provider’s clinical judgment. Together, Screen. Test. Treat. turns routine intake into a practical chronic-risk system.
Each DTC program is turnkey, clinic-friendly, and designed to be deployed with minimal disruption. Programs may be implemented inside a single exam room and can support existing primary care, specialty care, chronic-care, mobile, remote, and wellness workflows.
The goal is simple: help providers find hidden risk earlier, document it more clearly, act on it faster, and help patients avoid preventable complications before damage becomes irreversible.

Programs by Category
Neurological Risk
Cognitive decline, neuropathy, fall risk, and sensory impairment may develop silently or be underreported before complications become obvious.
Cardiometabolic Risk
Cardiovascular disease, PAD, and autonomic dysfunction frequently progress without warning. Early screening may help identify risk earlier and support timely intervention.
Behavioral Health & Chronic Disease Risks
Depression, diabetes distress, and behavioral health conditions directly worsen chronic disease outcomes. Early identification enables timely support and coordinated care.
Pain-Related Risks
Chronic foot and lower extremity pain is a recognized disease state and early indicator of neurological and vascular compromise. Early identification and systematic monitoring reduces the risk of wounds, amputations, and irreversible damage.

Built for Scale
DTC programs are available as individual clinic implementations or as a licensed system for medical groups, MSOs, ACOs, and healthcare entrepreneurs building chronic care networks. Organizations looking to deploy the full DTC model across multiple locations can explore our License & Partnership Models.
DTC recommends a trusted third-party ancillary billing partner for therapy programs and multi-code encounters to ensure full reimbursement integrity.
